Quest markers in both quests are setup wrong. When speaking to the doctor the marker stays at the hospital and after speaking to the lizardman and being sent to level 3 of the sewers it keeps the marker on his position in level 2 of the sewers.

I agree with regards to the quest indicator on the doctor. I've changed that to be in the barracks once you have him with you.
As for the "Forktongue Language" indicator, I would argue that it should remain on Vorbis as he is where you go to turn in the quest. It is consistent with other quests such as collecting items from creatures in that the indicator remains on the NPC expecting to receive the items.
