You used to get double the intended life steal from gems, this was a bug and is now fixed to be correct.
If you can give screenshots/examples of situation where it doesn't match the tool tip please let me know.

I tested this earlier with Qao.
If you have two Almandine gems in a 2H melee weapon(in this case two 38 health on hit gems in the L40 2H sword), it will only give you health equivalent to one Almandine gem on hit; therefore, half of the intended health on hit.
"Rolled" weapons(weapons that have no gem slots but affixes) work normally, even with a double health on hit roll.

I have a Wretched Hammer with two life on hit rolls (75 when added together) and Qao's combined gems are 76 health on hit. However my abilities and attacks were healing me for the correct amount whereas his gems were only healing him for half.
