I was running vault dungeon and swapping between dual wielding and my 2 handed sword mercy blade 

And On One Of the swaps it deleted my 2nd wrist weapon and placed the other one in my inventory.

It was a refined copper wrist blade if you guys can reimburse me im not looking for anything else i just want my wrist blade back please

Log off/on always makes the item re-appear.  Hopefully this works for you as well.  And apologies for not reporting this myself the first time I saw it.

 

I've had items appear to disappear three or four times.  It seems to randomly (and very rarely) happen when multiple events are occurring as you move the item.  Doesn't appear tied to gear changes.   For me it has occurred during gathering/crafting while moving inventory from bag to BP or reverse.

 

Happened to me only once since the game went live.  A couple of weeks ago I was foraging while moving an item from BP to bag when I was attacked by a mob.  Dropped the item in bag, when fight finished it was nowhere.  After relog, there it was exactly where I put it.

Next time don't right click and select destroy

it seems like this is what happened even though u dont think so...try be more careful when swapping weapons

 Dont come onto my post assuming i just said i right clicked my blade and clicked equipped and both my claws didnt come back to my bag so please read and stop being stupid. By ur logic my blade shouldve been destroyed

Try what Irradiated suggested... If that doesn't work and you actually did accidentally destroy your own weapon then I doubt you will be getting reimbursed for human error.

 

Either way Copper Wrist Blades aren't hard to come by... Just get a new one if you haven't already.

Something similar happened to me last week while crafting large quantities of potions. Was moving the stacks between my bags and belt when 2 entire stacks disappeared.

 

Logging out and back in fixed it so as Irradiated said, give this a try and hopefully it will sort everything out for you.

sometimes happen when you try to put an item to inventory exactly same time as other is put there by system (like when reorganising inventory while crafting and crafted items appear in your inventory) that the item you wanted to put to the inv disappears. but iut just not shown. if you try to put something else to the exact place where it was, the item shows again then.

happens occasionaly to me as well but is hard to repeat this bug intentionally.

I continued to play after the happening and move stuff in and out of bag so i think its lost now that i relogged . Cause i was skinning stuff till i had a full bag then crafting.
