from recent experience i've noticed this happening, if anyone else can confirm please do:
anything that changes your character stats, i.e. equipment changes, buffs, ammo reloading
now (even though the change is displayed) has a delay of some 10 seconds before going into effect
so if, for example, you adjust your gear and buffs to hit a creature and attack right away, even though the numbers display correctly, the system is still using numbers from that 10 or so seconds ago
most noticeable because of this is ammo reloading - as a soldier, if my clip ran dry during combat, it would say "algore reloaded" (miss a turn) and i would get a new, full clip and keep firing - now i get a "combat unresolved" or am killed after multiple "algore reloaded" messages ...... the clips *are* subtracted from inventory, but because of the delay the stats from the ammo aren't registering right away
so 1 - i'll lose multiple clips from my inventory, gone
and 2 - all the missed turns gives the creature that random chance to get in a lucky hit
i don't know how this affects ammo from other classes

i'll create a quick squad and attack...and get the "invalid squad" notice, unless i wait some time to use it
so for now, until this is fixed or for whatever reason stops happening, be sure to wait a bit before attacking ... and watch your ammo
