A simple prevention of password attacks:

1) Add another string to each player, to be used INSTEAD of his ingame alias for login purposes. Not displayed to anyone, ever.

2) After 1) is done, it is reasonable to have a system that locks any account after X consecutive failed logins, because the system then cannot be abused to lock somebody on purpose.

It would greatly reduce the amount of successful password guesses (although people who will have password "password" or "1234" would most likely not bother to change their login name, anyway).

Something like 10 attemps is reasonable IMHO... Soon enough that it doesn't really give the attacker too much opportunity, but late enough to make a sloppy typist realize he's really doing something wrong :)
