That determines the maximum number of combat rounds you can have.
For some elite creatures you *may* need more than 20 rounds, but normally the 20 stamina button is fine.

Yes, you dont lose/gain xp or gold, you just lose that stamina.
And yes, the default attack is 20 stamina.
P.S. If you're attacking creatures and wasting 20 stam to end in a draw, you either need better equipment or you should be attacking lower level creatures until you can beat the others.
P.P.S. When you level up, PUT ALL YOUR SKILL POINTS INTO DAMAGE.
Just saving you a little frustration later in life.
