MY thoughts:
All successful pvp attacks should take gold, exp and pvp points. Pvp without risk isn't pvp as we know it.
All attacks should have the option to be bountied.
Pvp point transfer should be removed from the bounty board completely.
Gloat and honor should be removed from the game.
Pvp rewards should be removed too. (or a tkp type system introduced if they must stay)
ive mentioned a tkp style system for pvp rewards before and it got shot down by most everyone for the simple fact that it would only promote volume of attacks and not skill. personally i like that idea and think if done right it could work quite well

How to Fix the Ladder
1. Implement a PvP floor. If you're under level 25 and you haven't been in the game for at least a month, you can't have PvP rating. Period.
2. Level bands of the following: 50-75, 76-99, 100-149, 150-199, 200-250, 251-324, 325-399, 400-499, 500-649, 650-849, 850-1000.
This breaks at the following gears:
Epics face anything up to KAX.
KAX does not have to face fushi.
Fushi does not have to deal with skaldir.
Skaldir and Sekhs don't have to fight demon queen or bahmou.
Mar and the other midrange armor sets aren't paired off versus opticor and dark blade/broken fang.
400 gear et al does not have to face the trident, Spirit Tainted, or the 500 armor gear.
Nuryns, Shu Zi, and Balaurs aren't faced off versus Ormarrs, Machteld, and Marshman.
Agiads doesn't have to compete with Clotilde or other 850 gear.
3. If you appear on the bracket's ladder, you are eligible to be faced off by ANYONE also on the ladder. If you've been targeted and defeated by any individual in the last 24 hours, you have the ability to hit them back to regain your rating. If other players have already taken that rating away, you get a portion of it, to an absolute minimum of 50%. ALL ATTACKS REMAIN BOUNTYABLE. However, no one on the ladder can stam anyone out of their normal pvp range who is NOT on the ladder, and if a player is not present on the ladder they cannot hit anyone on the ladder outside their normal pvp range.
4. All ladders will be reduced to 25 players, as the total number of eligible ladder players would be over 275 at this time.
5. Rating is still lost on the board, at a minimum of 20 rating for 10 hits. If a player bounties someone, they lose the ability to regain any rating they lost. However, if a bounty hunter is within two brackets of you below level 400 or within 3 brackets of you above level 400, rating exchange occurs as it does prior to these updates.

yep.
Only weird thing with this is the bracket where fushi does not have to face Skaldir. I've never, ever actually seens someone wearing a good Skaldir setup in my range.
Also, one problem is the introduction of new gear. Basing level ranges around gear would eventually mean that the ranges would become obsolete.

yes it is bad that higher level players don't have much chance now...Right now, the higher level player in the top 50 is level 471. Nothing else to say.
But it is interesting to see that a player with 1500+ rating can be on BB for 30 min without being attacked..
I hope that with few more fixes we'll actually be able to PvP

Remove Gloat and Honor
No PvP transfer on the BB unless they are in PvP range.
Same PvP attack range and rules. Maybe graduated increases as needed thats still fair to those being attacked, though if you just capture highest PvP Rating in a 50 level range it wouldnt matter.
Make level Brackets that only highlight the Top PvP points for those in that bracket. Simple.

It seems THAT counts as my one 24 Hour Win... as I just hit him and received NOTHING. and he has a rating of 1,225 and I have a Rating of 1,382.
My Point now a player can Gloat you then LOSE on purpose so you cant hit them back and gain rating.

Who in the hell came up with this 24 hour rule... surly no one who has a clue about PvP and Epic battles between PvP Players. Its bad enough you have to wait 1 hour.

wut? 24 hour rule is great. It's to prevent excessive trading. Perhaps you should think about the system as a whole? Perhaps you could suggest changes, there's a very obvious one that could be done in order for the 24 hour to be on each player's attack.
And stop using large font sizes.
24 hour sucks.. if you PvP.
Grab some glasses I did suggest changes just above the one I just posted.
I am thinking about´the system as a whole, if you ever PvPed, and are hitting one who would drop you 5 levels for a single attack then you would have a clue about why the the 24 hour rule sucks for those who like to do their own dirty work.
I speak for those who PvP, not just talk about it.. or do it on occasion or need a guild of killers to speak and fight for them.. Player verses Player...

On a Side NOTE.. I was 100 stammed and Gloated for 130 PvP.. Then attacked again the next Hour.. and won the battle and gained 7 while Defending.

24 hour rule really really sucks, and should be removed, it puts the targets with higher pvp at a big risk they potentially cant get the stolen pvp back in a series of revenge hits.

In other words, if I hit someone, that is the rating Xfer possible for 24 hours. HOWEVER, they have to hit me back for their rating transfer possibilty to occur.
I'm glad to see that quite a few people approve of the basics I laid out. With regards to a "good" skaldir setup, I can think of and have built a few. I also realize that they bracket suggestions aren't perfect. They were guidelines for where I, as an informed member of the PvP community, think that gear should be separated. For example, everyone is going to agree that level 99's should not have to face the epics at 100. If we can agree that the guidelines themselves are worth implementing, I'm more than happy to have a discussion about where exactly the brackets should be fixed at.
I'd also like to point out that if more than one perspective about where a particular bracket should be placed at exists, that's a great argument for having floating brackets. I had a pretty hard decision about whether to break at 819 (Imperial Demon) versus 850 (Acherons, and some suicide gear). Also, gear like harvester and Red Annis makes a strong argument for breaking lower.
