The idea: a heavy questing area with subareas. however the subareas are only accessible if you have done the quests. The quests would raise by level, somewhat starting at 3-4k up to 10k or higher. You finish quest 1, you get a bound key-like reward that allows you to start quest two and maybe 3. Once you do these you get rewards that combine to a new key to give you access to higher areas/other islands and to all areas below. So in the end you have a nice quest tree with nice rewards (SP, maybe new gear – hint: some mage ammies, base stats, maybe some item-bound skills becoming regular ones etc). These quests can include everything, normal kill counts, kill some smaller critters like locustas (parasites), location runs bound with riddles, a dark maze with moving locations, quest item drops, use of current crafted and sold items (like give some vials or forged weapons to help the good guys).

A nice storyline would be great, some person asks you to invest parts of the new area, gives you some chores and tells you a nice story. You get sent to other npcs that do basically the same and in the end you have to face some kind of boss creature like lord na'gath or the blood lord. The critter should include assassinate, zaahar shield and maybe a new skill, that heals the creature up by 50% or 100% if you can't kill it within 2-4 minutes or a longer time span, depends on the creatures stats and beta testing. And the catch about it: no safeheaven or mana shrine in the realm (something like the top floor of a tower, 1x2, 2x2 squares large, whatever, just really small but a bit annoying to get up there). The idea behind fighting such a boss creature is that the player should be strong enough to kill the creature before the timer runs out, or do it with a group and obviously give a great reward other than xp.
